频域光学相干断层扫描测量青光眼视网膜神经纤维层厚度的可重复性研究

Reproducibility of Retinal Nerve Fiber Layer Thickness Measured by Spectral-domain Optical Coherence Tomography in Glaucoma

摘要: 目的 利用频域光学相干断层扫描(SD-OCT)对青光眼患者盘周视网膜神经纤维层(RNFL)厚度进行观察测量,比较非Follow-up模式和Follow-up模式测量结果的可重复性。方法 连续收集就诊于中国医科大学附属第一医院的青光眼患者38例,随机选择患者一侧眼进行测量。利用SD-OCT分别采用非Follow-up模式和Follow-up模式对盘周RNFL厚度进行测量,比较2种检查模式的组内标准差(Sw)、变异系数(CV)和组内相关系数(ICC)。结果 非Follow-up模式测量RNFL厚度,Sw为3.67 μm(视盘全周)~10.58 μm(鼻侧象限),CV为2.60%(视盘全周)~10.09%(鼻侧象限),ICC为0.768(鼻侧象限)~0.974(视盘全周)。Follow-up模式测量RNFL厚度,Sw为1.42 μm(视盘全周)~3.31 μm(鼻上扇区),CV为1.27%(视盘全周)~3.00%(鼻侧象限),ICC为0.982(鼻侧象限)~0.996(视盘全周)。Follow-up模式测量的重复性优于非Follow-up模式。结论 利用SD-OCT测量盘周RNFL厚度,Follow-up模式的可重复性优于非Follow-up模式,达到了临床应用标准,可用于临床青光眼的检查和长期随访。

Abstract: Objective To compare the reproducibility of retinal nerve fiber layer(RNFL)thickness measured by No Follow-up and Follow-up patterns of high-resolution spectral domain-optical coherence tomography(SD-OCT)in glaucoma. Methods A total of 38 eyes from 38 patients with glaucoma who admitted to the First Hospital,China Medical University were sequentially included. RNFL thickness was measured by No Follow-up or Follow-up patterns of SD-OCT. Within-subject standard deviation(Sw),coefficient of variation(CV),and intraclass correlation coefficient (ICC)using the two measuring patterns were assessed. Results For RNFL thickness measured by No Follow-up pattern,Sw was 3.67 μm(global average,G)to 10.58 μm(nasal,N),CV was 2.60%(G)to 10.09%(N),and ICC was 0.768(N)to 0.974(G). For RNFL thickness measured by Follow-up pattern,Sw was 1.42 μm(G)to 3.31 μm(superior-nasal,NS),CV was 1.27%(G)to 3.00%(N),and ICC was 0.982(N)to 0.996(G),respectively. Follow-up pattern had higher reproducibility for the measurements of RNFL thickness than No Follow-up pattern. Conclusion For the measurements of RNFL thickness,Follow-up pattern has higher reproducibility than No Follow-up pattern,indicating that RNFL thickness measured by Follow-up pattern of SD-OCT is a reliable tool for assessing longitudinal change and follow-up in glaucoma.
