Effect of Contralaterally Controlled Functional Electrical Stimulation of Upper Limb Motor Function in Patients with Hemiplegia

摘要: 目的 观察对侧控制型功能性电刺激(CCFES)对急性期及亚急性期偏瘫上肢运动功能的影响。方法 将 48 例脑卒中患者随机分为对照组和实验组,每组 24 例。2 组均接受常规药物治疗和康复训练 4 周(6 d/周),所有患者分别于治疗前和治疗 后 采 用 Fugl - Meyer 运 动 功 能 评 定 量 表(FMA)、Wolf 运 动 功 能 测 试 量 表(WMFT)、腕 关 节 背 伸 的 主 动 关 节 活 动 度 评 测(WEAROM)和改良 Barthel 指数(MBI)进行评定,对结果进行统计分析并比较其疗效。结果 治疗前 2 组间各测评值差异无统 计学意义。治疗 4 周后,2 组的测评值 FMA、WMFT、WEAROM 和 MBI 组内比较差异有统计学意义(P < 0.05),组间比较 FMA、 WMFT 和 WEAROM 差异亦有统计学意义(P < 0.05)。结论 CCFES 能有效地改善急性及亚急性期脑卒中患者上肢运动功能。

Abstract: Objective To investigate the effect of contralateral controlled functional electrical stimulation(CCFES)on the upper limb motor function in patients with acute and subacute hemiplegia. Methods Forty-eight patients with cerebral apoplexy were randomly divided into con trol,neuromuscular electrical stimulation(NMES)experiment,and CCFES experiment groups,with 24 patients in each group. The two groups re ceived routine drug treatment and rehabilitation training. For 4 weeks(6 days/week),all the patients were assessed before and after treatment by us ing the Fugl - Meyer locomotor rating scale (FMA),Wolf motor function test (WMFT),wrist extension active joint activity evaluation(WEAROM),and modified Barthel index(MBI). Results were statistically analyzed,and curative effects were compared. Results Before treat ment,no significant differences in FMA,WMFT,WEAROM,and MBI scores were found between the two groups. At the fourth week after the in tervention,the FMA,WMFT,WEAROM,and MBI scores were all significantly increased(P < 0.05). The FMA,WMFT,and WEAROM scores were all higher in the treatment group than in the control group,and the differences between the two groups were significant(P < 0.05). Conclusion CCFES can effectively improve the upper limb motor function in patients with acute and subacute stroke.
