血清胱抑素C、血尿酸水平与冠状动脉粥样硬化性心脏病患者发生急性冠状动脉综合征的相关分析

Correlation of Serum Cystatin C and Uric Acid with Acute Coronary Syndrome in Patients with Coronary Heart Disease

摘要: 目的 研究冠状动脉粥样硬化性心脏病(CHD)患者血清胱抑素C(Cys-C)血尿酸(UA)水平与急性冠状动脉综合征(ACS)发病的相关性。方法 选取2013年10月至2015年1月我院心内科住院,经冠状动脉造影确诊的CHD患者183例,根据是否发生ACS分为2组:ACS组和非ACS组。收集患者的临床资料,对各项临床指标与ACS的关系进行相关分析;进一步对ACS危险因素进行多因素logistic回归分析。结果 Cys-C、UA与ACS的发生正相关(均P < 0.001),左心室射血分数(LVEF)与ACS的发生呈负相关(P < 0.001)。ACS组患者中,Cys-C与UA水平呈正相关(P=0.003),Cys-C与LVEF呈负相关(P=0.038);受试者工作特征(ROC)曲线分析结果显示,血清Cys-C曲线下面积为0.710(P < 0.001,95%CI:0.635~0.784),血UA曲线下面积为0.736(P < 0.001,95%CI:0.665~0.808)。结论 UA和血清Cys-C水平对于预测CHD患者ACS发病有重要价值。

Abstract: Objective To analyze the correlation of serum cystatin C (Cys-C) and uric acid (UA) levels with acute coronary syndrome (ACS) in patients with coronary heart disease (CHD). Methods Between October 2013 and January 2015,183 inpatients were diagnosed with CHD using coronary angiography,and they were divided into the ACS and non-ACS groups. Data were examined for the relationships between the clinical indicators and ACS;multifactorial logistic regression analysis was used to identify risk factors for ACS. Results Cys C and UA were positively correlated with ACS (all P < 0.001),and left ventricular ejection fraction (LVEF) was negatively correlated with ACS (P < 0.001). Cys C was positively correlated with UA level (P=0.003) and negatively correlated with LVEF (P=0.038) in the ACS group. Receiver operating characteristic curve analysis showed an area under the curve of 0.710 for Cys-C (P < 0.001, 95%CI:0.635 -0.784) and 0.736 for UA (P < 0.001,95%CI:0.665 -0.808). Conclusion UA and Cys-C levels are useful predictors of ACS in patients with CHD.
