Correlation of Neutrophil to Lymphocyte Ratio and Distribution Width of Erythrocytes with an Acute Exacerbation of Chronic Obstructive Pulmonary Disease

摘要: 目的 探讨中性粒细胞与淋巴细胞比值(NLR)、红细胞分布宽度(RDW)与慢性阻塞性肺疾病(COPD)患者急性加重及病情严重程度的相关性。方法 收集2015年10月至2016年10月本科室住院及门诊COPD患者243例,其中因急性加重住院患者145例,门诊稳定期患者98例;选取同期健康体检者200例作为对照组。收集患者临床资料,检测血常规、动脉血氧分压(PaO2)、C反应蛋白(CRP)等相关指标。结果 COPD急性加重患者NLR显著高于COPD稳定期患者及健康对照组(P < 0.05);而COPD急性加重恢复期NLR明显下降。NLR与FEV1占预计值百分比(FEV1% pred)、PaO2、RDW及CRP存在相关性(r分别为-0.222、-0.265、0.290,P均<0.05)。与COPD稳定期及对照组患者比较,RDW在患者急性加重期明显升高(P < 0.05),但与患者PaO2、CRP、FEV1% pred无明显相关(r分别为-0.065、0.052、0.033,均P > 0.05)。结论 NLR与COPD患者急性加重及病情严重程度均存在相关性;RDW与COPD急性加重相关,综合应用NLR和RDW指标可以更准确评估COPD患者的病情。

Abstract: Objective To explore the correlation of neutrophil to lymphocyte ratio (NLR) and red cell distribution width (RDW) with acute exacerbations and severity of chronic obstructive pulmonary disease (COPD). Methods A total of 243 patients were enrolled from our hospital between October 2015 and October 2016. There were 145 inpatients with acute exacerbations and 98 outpatients with clinically stable COPD. Two hundred healthy subjects were selected as the control group. Clinical data were collected and routine blood parameters partial pressure of oxygen,and C-reactive protein (CRP) levels were recorded. Results The NLR of patients with an acute COPD exacerbation was significantly higher than that of patients with stable COPD and healthy controls (P < 0.05). There was a correlation between NLR and FEV1% predicted,PaO2,RDW,and CRP (r=-0.222,-0.265,0.290,P < 0.05). The RDW was significantly elevated in patients with acute exacerbations (P < 0.05),but did not significantly correlate with PaO2,CRP,or FEV1% predicted (r=-0.065, 0.052,0.033,respectively; P > 0.05). Conclusion There was a correlation between the NLR and an acute COPD exacerbation and the severity of the disease. Further,RDW was associated with an acute COPD exacerbation. Evaluation of a combination of both NLR and RDW could facilitate a more accurate assessment of the disease course of COPD.
