应用频域光学相干断层扫描分析高度近视患者黄斑区外层视网膜厚度

Outer Retinal Thickness in the Macula of Highly Myopic Patients Detected by Spectral-domain Optical Coherence Tomography

摘要: 目的 应用频域光学相干断层扫描(SD-OCT)定量分析黄斑区外层视网膜厚度,探讨黄斑区外层视网膜厚度的分布规律及其随屈光度数变化的规律。方法 收集2017年1月至2018年1月就诊于中国医科大学附属第一医院眼科门诊的患者和体检者作为受试者,共66人66眼,并且根据其等效球镜(SE)度数分为对照组(正视组,-0.25D ≤ SE ≤ +0.25D,29人29眼)和试验组(高度近视组,SE ≤-6.00D,37人37眼)。应用Spectralis SD-OCT对受试者的黄斑区视网膜进行扫描测量,比较2组受试者间及黄斑不同区域间外层视网膜厚度的差异。结果 黄斑中央区外层视网膜厚度显著高于黄斑中央区的上方、鼻侧、下方和颞侧外层视网膜厚度,差异有统计学意义(P < 0.001);高度近视组与正视组的中央区外层视网膜厚度无明显差异(P=0.917);高度近视组黄斑中央区的上方、鼻侧、下方的外层视网膜厚度低于正视眼组,差异有统计学意义(P < 0.05)。结论 黄斑区的外层视网膜厚度分布存在一定规律,黄斑中央区的外层视网膜厚度高于黄斑中央旁区。高度近视者黄斑中央区上方、鼻侧、下方的外层视网膜厚度低于正视眼者。

Abstract: Objective To quantitatively analyze the outer retinal thickness in the macula by using spectral-domain optical coherence tomography(SD-OCT). Changes in these patterns with myopic severity were also examined.Methods Sixty-six patients(66 eyes)were enrolled in this study. Based on the spherical equivalent(SE),29 patients(29 eyes)were included in the control group(normal group,-0.25D ≤ SE ≤ +0.25D)and 37 patients(37 eyes)were included in the study group(high myopia group,SE ≤ -6.00D). All patients were examined with the Heidelberg SD-OCT. Regional macular outer retinal thickness was compared between the control and study groups. Outer retinal thickness was also compared between different macular regions.Results Macular outer retinal thickness in the central region was significantly thicker than that in the superior,nasal,inferior,and temporal regions(P < 0.001). Macular outer retinal thickness in the central area was not significantly different between the control and study groups(P=0.917). In the superior,nasal,inferior,and temporal macular regions,outer retinal thickness was thinner in the study group than in the control group(P < 0.05).Conclusion Outer retinal thickness in the macula has a certain regularity. Outer retinal thickness of the central macular region is thicker than that in other macular regions. In the superior,nasal,inferior,and temporal regions of the macula,outer retinal thickness is thinner in highly myopic eyes than in normal eyes.
