肠杆菌科细菌mcr-1基因筛查及其与多黏菌素耐药表型的相关性

Screening for the mcr-1 Gene in the Members of the Enterobacteriaceae and Its Correlation with Colistin Resistant Phenotype

摘要: 目的 筛查沈阳地区肠杆菌科细菌mcr-1基因并进行分析,明确本地区mcr-1基因在肠杆菌科细菌中的携带率,携带mcr-1基因菌株的流行情况以及对多黏菌素的耐药情况。方法 收集沈阳地区5家三甲医院2013年至2015年临床分离的肠杆菌科细菌共计2 680株,采用PCR方法筛查mcr-1基因,结果阳性菌株用Sanger测序方法进行验证;用脉冲场凝胶电泳检测携带mcr-1基因的肠杆菌科细菌的同源性。采用微量肉汤稀释法进行多黏菌素的药敏试验。结果 沈阳地区2 680株肠杆菌科细菌携带mcr-1基因者共26株(0.97%),其中大肠埃希菌20株(1.4%)、肺炎克雷伯菌6株(0.8%)。携带mcr-1基因的大肠埃希菌和肺炎克雷伯菌对多黏菌素的耐药率分别为65%(13/20)和83.3%(5/6),不敏感率为95%(19/20)和100%(20/20);对照组的40株大肠埃希菌和12株肺炎克雷伯菌均对多黏菌素敏感。中国医科大学附属第一医院携带mcr-1基因的19株大肠埃希菌的脉冲场凝胶电泳结果为无关,不具有同源性。结论 沈阳地区发现了少量携带mcr-1基因的大肠埃希菌和肺炎克雷伯菌;携带mcr-1基因与多黏菌素耐药密切相关;本地区携带mcr-1基因的肠杆菌科细菌未出现医院感染的爆发流行。

Abstract: Objective To quantitatively screen the presence of mcr-1 gene in Enterobacteriaceae in Shenyang,and study its resistance status to Colistin. Methods A total of 2 680 strains of Enterobacteriaceae were collected from five top three hospitals in Shenyang from 2013 to 2015. The mcr-1 gene was screened by PCR amplification and the amplicons were verified by sequencing. Pulsed-field gel electrophoresis was performed on strains harboring the mcr-1 gene. The susceptibility of mcr-1 positive strains to colistin was tested by broth microdilution method. Results Twennty six(0.97%) out of 2 680 strains were found positive for mcr-1 gene,including 20 strains of E. coli (1.4%) and 6 strains of Klebsiella pneumoniae(0.8%). The resistance rates of the mcr-1-positive strains of E. coli and K. pneumoniae to colistin were 65%(13/20) and 83.3%(5/6),respectively. Their respective susceptibility rates were 95%(19/20) and 100%(6/6). In the control group,all the strains were found to be sensitive to colistin. The results frompulsed-field gel electrophoresis suggested that 19 of the mcr-1-positive E. coli strains collected from the First Hospital of China Medical University were non-homologous. Conclusion These results indicate a low occurrence of mcr-1 gene in the enterobacterial isolates of Shenyang and the close association of the mcr-1 gene with colistin resistance. As suggested from the results of pulsed-field gel electrophoresis,there has been no outbreak of nosocomial infection caused by mcr-1 carrying Enterobacteriaceae.
