326株念珠菌血症病原菌种分布和药敏结果分析

Analysis on Pathogen Distribution and Drug Susceptibility in 326 Cases with Candida Bloodstream Infection

摘要: 目的 了解念珠菌血症的病原菌和科室分布及药敏结果,为临床念珠菌血症病原学诊断和合理使用抗真菌药物提供依据。方法 对我院2014年1月至2017年6月间血培养分离的念珠菌进行菌谱分布及耐药性分析,对相关患者进行临床资料的回顾性分析。结果 从17 391份送检的血培养标本中共分离培养念珠菌326株(1.9%)。我院念珠菌血症好发于>65岁的老年患者(39.6%),男性居多(61.35%)。念珠菌血症患者主要来自外科重症监护病房(23.93%)和胃肠肿瘤外科(15.03%)等外科手术科室。病原菌主要为近平滑念珠菌(133株,40.80%)、季也蒙念珠菌(120株,36.81%)和白念珠菌(33株,10.12%),少见念珠菌包括克柔、葡萄牙和希木龙念珠菌。所有念珠菌对5-氟胞嘧啶和两性霉素B均敏感,而白念珠菌、光滑念珠菌、热带念珠菌和希木龙念珠菌对唑类药物出现不同程度耐药。结论 念珠菌血症在医院感染中呈增多趋势,且部分菌株出现了对氟康唑、伏立康唑和伊曲康唑耐药的情况,应引起临床医生的足够重视。

Abstract: Objective To investigate pathogens and their departmental distribution and analyze the susceptibility of anti-fungal drugs against Candida bloodstream infections to establish a basis for clinical anti-fungal therapy and rational drug use. Methods Laboratory data of Candida isolated from bloodstream infections were retrospectively analyzed in the First Hospital of China Medical University from January 2014 to June 2017. Results A total of 3 289 stains were isolated from 17 391 blood culture specimens that included 326 strains of Candida. In our hospital, candidemia usually develops in patients aged >65 years(39.6%) and mostly in men(61.35%). Infected patients mainly presented from the surgery intensive care unit(23.93%), gastroenterological surgery(15.03%), and other surgery departments. Among the 326 strains, Candida parapsilosis(40.80%), C. guilliermondii(36.81%), and C. albicans(10.12%) were the most common, whereas C. lusitaniae, C. krusei, and C. haemulonii were the least common. All 326 strains were highly sensitive to amphotericin B and flucytosine, with the sensitivity rate of 100%. However, the sensitivity rates, especially those of C. albicans, C. glabrata, and C. tropicalis, to voriconazole, fluconazole, and itraconazole were low. Conclusion The primary Candida pathogens isolated from bloodstream infections were C. parapsilosis and C. guilliermondii. Their sensitivities to fluconazole, voriconazole, and itraconazole are decreasing.
