波前像差引导的准分子激光上皮瓣下角膜磨镶术后早期客观视觉质量的临床观察

Early Visual Quality after Wavefront-guided Individualized Laser Epithelial Keratomileusis Ablation for Myopia

摘要: 目的 评估行波前像差引导的准分子激光上皮瓣下角膜磨镶术(LASEK)的患者术后早期视觉质量。方法 对我院2017年5月至2018年2月采用波前像差引导的LASEK手术的56例(110眼)临床资料进行回顾性分析。采用欧卡斯测量仪双通道视觉质量分析系统(OQASTMⅡ)评估入选患者术后的调制传递函数截止空间频率(MTF cutoff)、斯特列尔比值(SR)及客观散射指数(OSI)。应用重复测量方差分析的方法分析患者术前与术后1、3及6个月视觉质量的变化。结果 术后不同时期视力和等效球镜度数与术前相比均有统计学差异(P < 0.001)。术后1个月的MTF cutoff较术前下降(P=0.014),术后3个月和6个月时该数值逐渐恢复,且较术前升高,差异有统计学意义(P < 0.001)。术后各时间点的SR与术前比较无统计学差异(P=0.096,P=0.146,P=0.055)。术后1个月和3个月OSI较术前略有提升(P < 0.001,P=0.032),术后6个月时较术前降低,但差异无统计学意义(P=0.252)。结论 采用波前像差引导的个性化切削模式LASEK的患者术后早期视觉质量较术前提高。OQASTMⅡ能够对准分子激光术后患者的视觉质量进行定性和定量分析,有助于近视矫正患者术前及术后视觉质量的随访观察。

Abstract: Objective To evaluate early visual quality in patients with myopia after wavefront-guided individualized laser epithelial keratomileusis (LASEK)ablation with the double-pass optical quality analysis system (OQAS). Methods In all 56 myopic patients (110 eyes)who underwent wavefront-guided individualized LASEK ablation between May 2017 to February 2018 were selected for this retrospective case study. All these patients were followed up for modulation transfer function cutoff frequency,Strehl ratio,and objective scattering index in the OQAS 1 month,3 months,and 6 months postoperatively. Results There were significant differences in visual acuity and spherical equivalent degree at each postoperative visit when compared with the preoperative period (P < 0.001). Modulation transfer function cutoff was significantly lower 1 month postoperatively (P=0.014),after which it increased at 3 and 6 months postoperatively (P < 0.001).There was no statistically significant difference in Strehl ratio between the preoperative and postoperative follow-up (P=0.096,P=0.146,P=0.055). Objective scattering index at postoperative 1 month and 3 months was higher than it was preoperatively (P < 0.001,P=0.032),but it was lower than the preoperative level 6 months after the operation,with no statistically significant difference (P=0.252). Conclusion Wavefront-guided individualized LASEK ablation provides a high optical quality compared to that before surgery. The OQAS can be used as a comprehensive tool to evaluate the postoperative visual quality before and after LASEK ablation.
