语前聋儿童人工耳蜗植入术后疗效分析

Therapeutic Effect of Cochlear Implantation in Prelingually Deaf Children

摘要:

目的探索语前聋儿童人工耳蜗植入术后的疗效及相关影响因素。方法对48例行人工耳蜗植入术的语前聋儿童的家长及康复教师进行问卷调查,调查的内容包括患儿的基本情况、术前评估、植入装置、植入时年龄、术后康复时间、康复模式、术前是否佩戴助听器、言语发展水平满意度及听觉行为分级标准表等。根据听觉行为分级标准(CAP)对患儿听觉感知能力进行评估,并分析病因、性别、病程、康复模式、术前是否佩戴助听器等因素与CAP的相关性。结果语前聋儿童人工耳蜗植入术后3年能不借助唇读即与人交谈的达到51%,术后5年以上不借助唇读即与人交谈的达到58%。以植入后时间为协变量,采用协方差分析结果显示:病程(P<0.0005)、康复模式(P<0.0005)与CAP显著相关,性别和病因与CAP无关。结论人工耳蜗植入对语前聋儿童的听觉言语发育有明显的效果。病程限制在2年以内,更有利于人工耳蜗植入儿童的听觉言语康复。对植入人工耳蜗的儿童选择康复机构的康复模式更有利于其听觉言语康复。

Abstract:

Objective To explore the therapeutic effect of cochlear
      implantation and the related influencing factors in prelingually deaf
      children.Methods The parents or the teachers of 48 prelingually deaf
      children undergoing cochlear implantation were asked to complete a
      questionnaire to answer the questions about preoperative
      evaluation,implantation equipment,age at operation,postoperative
      rehabilitation period,rehabilitation mode,whether wear a hearing aid
      fitting before operation,satisfactory degree of language development after
      operation,categories of auditory performance(CAP),and so on.The aural
      ability was evaluated by CAP,and the correlation of CAP with
      etiology,sex,course of disease,rehabilitation mode,and whether wear a
      hearing aid fitting before operation was analyzed.Results Up to 51% of the
      prelingually deaf children could understand conversation without
      lip-reading 3 years after operation,and the number reached 58% 5 years

      results of covariance analysis suggested that course of disease and
      rehabilitation mode were significantly correlated with CAP(both
      P<0.05),but no correlation was found between sex and etiology or
      CAP.Conclusion The therapeutic effect of cochlear implantation on hearing
      is satisfactory in prelingually deaf children,and limiting the course of
      disease within 2 years is of benefit to hearing and language
      development.The rehabilitation mode may affect hearing and language
      development in prelingually deaf children undergoing cochlear
      implantation.
