影响种植体存留率的相关危险因素分析

Analysis of Related Risk Factors for Implant Survival

摘要: 目的 分析并评价相关危险因素对种植体存留的远期影响。方法 选择1036例患者,共植入2041枚种植体。以种植体为观察单位,记录并评价一系列与种植体脱落密切相关的患者自身、种植体以及手术和修复相关因素。结果 总的种植体存留率为97.75%。单因素分析结果中有意义的变量包括年龄、骨密度、牙位、糖尿病、吸烟习惯和埋置方式(P<0.05)。logistic多因素回归分析发现除年龄和牙位,其余因素仍具有统计学意义。性别、种植体的直径和长度、植骨手术、缺牙时间和修复时间均不是影响种植体存留的危险因素(P>0.05)。结论 影响种植体存留的危险因素多为患者自身因素,建议临床医生应多注意对种植手术适应证的把握,对高风险患者应给予充分的术前交代。

Abstract: Objective: To analyze and evaluate the risk factors associated with implant long-term survival. Methods and materials: 2041 implants placed in 1036 patients between January 2005 and June 2011 were enrolled in this retrospective study. Dental implant was regarded as an observed unit in this study. Clinical records were evaluated for implant failure in terms of a series of patient and implant-related, surgery and prosthese-dependent factors. Results: By the end of 2012, forty-six of the taotal 2041 implants placed were lost, sugggesting an overall survival rate of 97.75%. Risk factors showing significant impact on failure on the basis of univariate analyses were age, bone mineral density, tooth position, diabetes mellitus, smoking habit and surgical protocol (all p<0.05). Logistic multiple regression analysis confirmed that most of the factors remained significant except for age and tooth position. No significant influence was observed on implant survival with regard to gender, implant diameter and length, period for loss of tooth and loading protocol (all p>0.05). Conclusion: Patient-related factors mainly affect implant survival. Clinicians should pay more attention to the mastery of the indications of implant surgery, sufficient preoperative explanation should be performed to patients related to high risk, and relevant measures should be taken actively with regard to these factors can be controlled artificially.
